Use the provider messaging feature in your patient portal to share the form or document you need completed. Attach the document directly to your message.
You message your provider by logging into MyVisana patient portal at www.myvisana.com and clicking on "Messages" on the left hand side menu. From there, in the center of the screen, you’ll see an option to “Message your Provider” and to the right, click the “New Message” button to send a message directly to your provider. If you need assistance with this, you can let us know you need help and a care coordinator will assist you with contacting your provider.

Submitting paperwork does not guarantee your provider will be able to approve or complete it. Your provider will review the request at your appointment and determine what they can provide based on your medical situation.
