After you schedule your first appointment, you’ll be directed to the MyVisana Patient Portal to log in. Once you’re signed in, you’ll be taken directly to the Patient Profile section, where you can enter a few important details — like your contact information, insurance, and photo ID.
If you need to come back to it again, you can continue or update your p-re-visit patient profile here.
After completing your Patient Profile, you’ll move on to the Health History form to share more about your personal and family medical history.
If you’re unable to finish either form right away, you can log back in at any time to complete them. We’ll also send you reminders if any forms are still incomplete before your visit.
Completing both forms ahead of time helps your provider and Care Team prepare — so your visit can focus fully on you and your care.
