How to Upload Your ID

Step-by-step guide to upload your photo ID from your computer or smartphone to the MyVisana Patient Portal before your first visit.

Before your first visit, we’ll need a copy of your valid (non-expired) government-issued photo ID: state ID, drivers license or passport are all accepted. You can upload it from your computer or smartphone — whichever is easiest for you.

Many patients find it simplest to take a photo and upload it directly from their smartphone. Here’s how:

  1. Go to your patient profile in MyVisana

  2. Click or tap “Add,” then choose to take a photo of the front of your ID or upload an existing image. It must me a jpeg or png image format - other formats like PDF won't work.

  3. When you’re done, tap “Save” to upload your photo ID.

  4. If your insurance changes, upload new photos of your card, and provide the updated details when you book your next follow up appointment.

Once it’s uploaded, you’re all set — no need to send it separately.
